Banner Forms: Reporting a Problem

Details

To report a problem with Banner Forms (e.g., error messages or system failures), send email to help@dartmouth.edu and provide the following information:

  1. Your full name as it appears in the Dartmouth Name Directory (DND).
  2. Your NetID.
  3. The version and operating system of the machine you are using (e.g., Mac OX 10.9.x, Windows 7).
  4. The web browser and version you are using (e.g., Internet Explorer 11, Safari, 9.x, FireFox 47.x, etc.).
  5. The name of the Banner Form (window) you were accessing when the error occurred.
  6. The exact error message that appeared.
  7. Indicate whether this is affecting only you or multiple users within your area/department.
